You will need a header image, which is the same as the other pages on your website. You will also need a title and then some details about the form. The details section is a wysiwyg editor so you can add just about anything to it.

Last you will need the shortcode section filled out. There will be a place for a main title, then an option to add a link to a product on your site. This is helpful for when you need to have paid featured forms. Simply create the product, in this case it would be some sort of registration fee to fill out the form, then add the link to that product here. Now people will know before they fill out the form that they need to pay the fee first.
